Get a Police Report

Start by completing the report copy release form.

Police Incident Report Copy Release Form

  • Include your case number, mailing address, email address and/or fax number on the form. 
  • If you do not have the case number, the Records Manager can retrieve it for you. A case number is not required to make a request if the time, date, location, type of report, and name of person(s) involved is listed on the form. 
  • Send in the the form (see below) along with the fee of $10 or pay online. 
  • You must pay the fee when you submit your request. 
  • We accept credit cards, cash as well as checks and money orders: Payable to “UC Regents”.
  • No fee is required for UCSF departmental requests.

What happens next? 

Once we receive your request form and fee the report may be sent to you either electronically, as a PDF, by fax or U.S. mail.

You can expect your police-report copy to be sent within two working days after we receive your request.  

  • Police reports may not be available for immediate pickup.  
  • A report may not be released until the incident investigation is complete.
