Lost or Stolen Cards

Please report lost or stolen UCSF ID cards immediately to keep them from being used by unauthorized individuals.  The fastest method of reporting a lost or stolen ID Card is to call 415/476-1414 (24/7)

Replacing Your ID Card

If you need a replacement UCSF ID Card you must contact your home department or school HR rep/ID Worker and let them know you need a replacement card and why. Your home department or school determines whether they will pay for your card or if you will pay for the card as WeID does not subsidize lost, stolen or non-working ID Cards.

If your home department or school will pay for the replacement, they need to enter the recharge account information into Hitachi prior to you going to a badging station to obtain the replacement card.

For NON-MEDICAL CENTER ID Card replacement appointments - use the Online Appointment maker.  Or You may call 415/476-2088, Option1 and leave a CLEAR message (name and best phone number) and your call will be returned as soon as possible.
