• UCSF Police Department: ID Cards

Many UCSF facilities use an electronic door locking system. This system relies on the use of ID cards to only provide access to those who are properly authorized. By obtaining and wearing the required ID card while on UCSF property, you can help enhance our overall security.  Upon being issued, a UCSF ID card is not activated.  The person’s home department can activate the card to open certain doors.

UCSF Policy on ID Cards

To Get an ID Card:

    1. First, get approval from your department representative (usually your HR representative).
    2. Your representative must enter your information into the campus computer system.
    3. Make an appointment.

      Medical Center personnel must contact Medical Center Human Resources or Medical Center Security for ID Cards.

      What to bring to your ID card appointment:

      • At least one valid government photo identification
      • Your home-department determines what Credentials display on your ID Card
      • Home-department enters up to three Credentials in the Campus Locator CLUP screen
      • Badging staff verifies Credentials on CLDS screen at card issuance
      • The $22 fee for new or replacement ID cards (your department may pay the fee by entering account information into the computer system).  We accept cash, checks made payable to UC Regents, Visa/Master Card.  Federal Cost Accounting Standards require that UCSF identification badges be charged to non-federal fund sources.  Fingerprinting can be charged to any fund source except one in the range of 21100 through 33999.

      To make an appointment, please contact the We ID Team directly at 415/476-2088.  If the call goes to voicemail, please leave a detailed message with your name and contact telephone number.  A member of the We ID Team will contact you by the end of the same business day to schedule your ID card appointment.

      Drop-in service for UCSF ID Cards is only available at the Parnassus campus location- (500 Parnassus Ave, Millberry Union Building, Level P-7). Customers may still schedule appointments by calling our customer service line at 415-476-2088.

      Priority will be given to customers with appointments and drop-in service completed on first-come, first-served basis.

      Credentials

      If a credential is required to appear on your UCSF ID card, the credential must be verified prior to issuance of the ID card. The credential will be added to the end of your last name.

      Examples:
      • John Smith, DDS
      • Jane Brown, MD
       

      Valid Forms of Identification

      • U.S. Passport or U.S. Passport Card
      • Driver's license or ID card issued by a State or outlying possession of the United States provided it contains a photograph or information such as name, date of birth, gender, height, eye color, and address
      • ID card issued by federal, state or local government agencies or entities, provided it contains a photograph or information such as name, date of birth, gender, height, eye color, and address
      • School ID card with a photograph
      • Voter's registration card
      • U.S. Military card or draft record
      • Military dependent's ID card
      • U.S. Coast Guard Merchant Mariner Card
      • Native American tribal document
      • Driver's license issued by a Canadian government authority
      • Permanent Resident Card or Alien Registration Receipt Card (Form I-551)
      • Foreign passport that contains a temporary I-551 stamp or temporary I-551 printed notation on a machine-readable immigrant visa
      • Employment Authorization Document that contains a photograph (Form I-766)
      • In the case of a non-immigrant alien authorized to work for a specific employer incident to status, a foreign passport with Form I-94 or Form I-94A bearing the same name as the passport and containing an endorsement of the alien's non-immigrant status, as long as the period of endorsement has not yet expired and the proposed employment is not in conflict with any restrictions or limitations identified on the form
      • Passport from the Federated States of Micronesia (FSM) or the Republic of the Marshall Islands (RMI) with Form I-94 or Form I-94A indicating non-immigrant admission under the Compact of Free Association Between the United States and the FSM or RMI

      UCSF Medical Center Employees or Affiliates

      Please contact your home department or UCSF Medical Center contact for further information and authorization to obtain a UCSF Medical Center ID card.

      When you have received authorization, contact the UCSF Medical Center Security Division at 885-7890 to get your ID card, The Medical Center ID Badging station is located at M-192 (Moffitt, 1st Floor, Room 192)

      To add proximity access to your UCSF Medical Center ID Card send an email to:

      The email should contain:
      • First and last name
      • UCSF ID number (begins with 02)
      • 8-digit number on back of UCSF Medical Center ID badge
      • Building and room number, plus actual access code (if known)

      UCSF Faculty, Postdocs and Staff

      Contact your manager, supervisor or PI to request authorization for a new or replacement ID card.

      UCSF Students

      New ID Cards

      Contact your school’s administration for information on receiving your UCSF ID card.

      Replacement Cards:
      • Get approval from your school’s administration to receive a new ID card (regardless of who is paying for the card).
      • If the school is paying for the card, a representative must enter the DPA and fund into the ICRD screen.
      • If you are paying for the card, bring written approval from your school’s administration authorizing a replacement card, with an appropriate signature.
      • Go to a service location and bring one acceptable form of identification.
      • Contact your school’s administration if you have questions.

      UCSF Contractors and Affiliates

      If you are a contractor, vendor or volunteer who is not paid through UCSF’s payroll, you must supply required information to the UCSF department that hired you. They will inform you when you can obtain your UCSF ID card.

      Our service locations close occasionally for training and meetings and for all UCSF holidays. Notice of closures will be posted on the door and will not occur when there are scheduled appointments.

       

      UCSF Police Department: How to Return a Card

      UCSF ID cards are the property of the University of California. Policy requires that departments collect UCSF ID cards from all persons who are discontinuing their association with the University. ID cards are then forwarded to any ID card service location, or interoffice mail to campus box 0238.
