Did the incident include the use of your credit or debit card without your authorization, or was you identity used?       Yes   No
Do you believe you are the victim of a crime because of your race, religion, sexual orientation, handicap, ethnicity or national origin?    Yes   No

Note: Filing a false police report is a crime! Every person who knowingly makes a false report that a felony or misdemeanor has been committed is guilty of a misdemeanor per 148.5 (a) of the California Penal Code.
