How to Obtain a Police Report

To request a police report:

Click the following link to complete a UCSF Police Incident Report Copy Release Form.

COSTCash, check, money order for $10.00, payable to UC Regents, must be paid at the time the request is submitted. (Fees will be waived for UCSF departmental requests.) 

Once payment is received, reports can be scanned and sent electronically as a PDF. Reports can also be sent by fax or U.S. Postal Service mail. On the Form remember to include your mailing address, email address and/or fax number. If you do not have the case number, the records manager can retrieve the case number for you.

Routine requests will be sent within two working days. Police reports may not be available for immediate pickup. Reports may not be released until the investigation is complete.
