UCSF Police Department Police Reports

To request a polices report:

Routine requests will be mailed within two working days. Police reports will not be available for immediate pickup. Reports may not be released until the investigation is complete.

Cost: Cash, check, money order debit/credit card for $5, payable to UC Regents, must be paid at the time the request is submitted. There is no fee for reports involving University-owned property or vehicles.

Completed request forms can be hand-delivered or mailed to:

Once payment is received, reports can be scanned and sent electronically as a PDF.  Reports can be sent through a fax or U.S. Postal Service mail.
