
To request a police report:
- Complete a UCSF PD Police Incident Report Copy Release Form.
- Include a mailing address. The records manager can retrieve the case number if it is not known.
Routine requests will be mailed within two working days. Police reports will not be available for immediate pickup. Reports may not be released until the investigation is complete.
Cost: Cash, check, money order for $5, payable to UC Regents, must be paid at the time the request is submitted. There is no fee for reports involving University-owned property or vehicles.

Once payment is received, reports can be scanned and sent electronically as a PDF. Reports can be sent through a fax or U.S. Postal Service mail.
