A: Have your supervisor of manager complete a Request for Sensitive Employee Fingerprinting Form (campus employee) or UCSF Medical Center Request for Fingerprint Form (medical center employee) 

 

 

Q: How do I know if I need my fingerprints taken for employment?
A: Most jobs that require fingerprints are well established and it is usually mentioned in the job announcement.  If you do not know, check with your supervisor or manager.

 

 

Q: What do I do if I need to have my fingerprints taken for a professional license?

 A: Obtain a Request for LiveScan Service Applicant Submission Form from the licensing agency if in California.  For out-of –State licensing you must obtain the appropriate fingerprint cards from the licensing agency.

 

 

Q: How do I get my fingerprints taken?

A: Call the LiveScan program at 476-2088 to schedule an appointment.

 

 

Q: How long does this take?
A: Taking your fingerprints will take 20 to 30 minutes.
